Recurrent
miscarriage in the first trimester of pregnancy is a devastating
problem which can and should be carefully evaluated, especially
since there are a number of treatable problems which can be detected
with a careful work-up. The known causes of recurrent miscarriage
fall into one of the following categories:
| 1)
Genetic
2)
Hormonal
3)
Immunologic
4)
Anatomic
5)
Infectious
6)
Unexplained |
A comprehensive
evaluation should include the following testing:
| 1) |
Chromosomal
analysis of both partners. |
| 2) |
Hormonal
testing to include thyroid, pituitary and ovarian function. |
| 3) |
Testing
for several known immunologic markers associated with recurrent
pregnancy loss. |
| 4) |
X-ray of
uterus (hysterosalpingogram). |
| 5) |
Sampling
of cervical secretions for presence of specific bacteria. |
One important
consideration is that the prognosis with this problem is extremely
good. Most couples with recurrent pregnancy loss will ultimately
carry successfully.
